Insurance 

 Appointments:
Regular appointments are scheduled Monday through Thursday between the hours of 8:00 a.m. and 4:45 p.m.  The office is closed on Friday.  Special appointment times can be made for emergencies or other unusual circumstances.

 Office Hours:
Monday        8:00 a.m. - 5:00 p.m.
Tuesday       8:00 a.m. - 5:00 p.m.
Wednesday   8:00 a.m. - 5:00 p.m.
Thursday      8:00 a.m. - 5:00 p.m.
Friday           Closed
 

Please make non-emergency calls during these hours.

If you reach the office answering machine, please be sure to leave a message including your name, telephone number and day and time of call. Calls will be returned as soon as possible during normal business hours.


For  
EMERGENCY calls - The answering service usually handles all off-hour emergency calls, but if you do reach our answering machine, please follow the instructions on the message and leave a message before completing the instructions given.. Emergency calls are handled as soon as possible.

 Fees:
If you wish to know the cost of your treatment in advance, please feel free to ask. Our fees for services rendered are commensurate with other podiatric offices in this area and are influenced by policies set by the Insurance Industry.

 Payments & Billing:
The patient (or legal guardian) is responsible for the total amount charged for services rendered. What you pay at the time of service is determined by what type of insurance coverage you have. After your insurance company responds to our billing, you will be sent a final statement. Balances are due within thirty days unless prior arrangements have been made.

If you do not have insurance coverage, please consult with our front-office staff so that they may present your options to you.

 Insurance:
We request that you become familiar with your insurance company's policies regarding podiatric care. If pre-determination is required it is your responsibility to obtain the proper forms before treatment is rendered.

Primary and secondary insurance forms are filled out and filed for our patients as a courtesy. We are ready and willing to provide whatever information you may need to obtain benefits from your insurer.
